|Description:||The North East Texas Center for Rural Community Health (The Center) is an organizational group within the University of Texas Health Science Center at Tyler (UTHSCT) for the purpose of 1) engaging in community health service activities within the Northeast Texas region and 2) enhancing, promoting, coordinating, and supporting faculty and student projects which are broad-based and intended to transform the regional healthcare delivery system through managed care for Medicaid recipients. The Center will manage approved Delivery System Reform Incentive Payment (DSRIP) projects to demonstrate effective approaches to achieve the health care system triple aims 1) improve access, 2) improve quality of care, and 3) reduce costs.|
Job Summary: The community health worker instructor will work with a multidisciplinary team that includes a program manager, training coordinator, and project coordinator. Primary responsibilities include assisting with the development and implementation of Texas Department of State Health Services approved training programs that emphasize and support the chronic care model, the patient centered medical home, and self-management principles leading to certification. This program will require the instructor to conduct face-to-face and online instruction using a standardized program.
The position reports to the Program Manager for this project.
|Duties:||A. Assist in the development of the community health worker training program|
 Obtain training and certification as an instructor of the program and maintain certification.
 Assist with writing and standardizing the community health worker program curriculum including continuing education curriculum.
 Complete periodic assessment of student knowledge, skills, and abilities.
 Report student barriers and successes to the program team
 Assist with the development of student remediation plans as needed
 Provide student mentoring
 May participate in site visits to observe CHWs in practice for training evaluation
 Remains current on educational theory, business practices, and training methods
B. Deliver the community health worker training program
 Conduct face-to-face and online community health worker education using the standardized program.
 Prepare and mail printed materials to community health worker students.
 Ensure CHW training supply inventory is stocked and adequate.
 Maintain accurate and timely documentation of all student interactions and outcomes.
 Performs other duties as assigned.
|Qualifications:||REQUIRED EDUCATION / EXPERIENCE:|
Bachelorís degree in health education, counseling, psychology, social work, execise science, community health or equivalent health-related education at an Associateís degree level, and two years of relevant experience required. is required. Current DSHS Certified CHW Instructor preferred. Knowledge of community resources and experience working with a variety of communtiy-based organizations is preferred. Demonstrated ability to use Microsoft Office PowerPoint, Word and Excel and various other software programs is required.
Standard office equipment will commonly be used within this position which includes the following office equipment: telephone, facsimile, copier, personal computer, printer, and audio/visual.
This position is generally in an administrative environment. Includes travel in between buildings and driving between cities.
